See cbl.abuseat.org. It's effectively a proxy DNSBL, and is more effective than any of the others. More effective than many of the more reasonable "combo" DNSBLs too.
Almost non-existant FPs.

I should also mention that OPM and PSS (originally osirus's open socks proxy BL) are also quite good.
